Graduates of a 5-year Bachelor of Science in Pharmacy, majoring in Clinical Pharmacy, have a unique opportunity to elevate their credentials and specialize in advanced patient care.

Enrollment is now ongoing for the 2nd Year of our Doctor of Pharmacy (PharmD) program.

Enrollment Requirements

For enrollees who have NOT ENROLLED in other schools after finishing the 5^th year BS in Pharmacy major in Clinical Pharmacy at 夜色视频:聽

1. Submit 2×2 picture
2. Update PSA Birth Certificate or Marriage Contract (if applicable)

For enrollees who have ENROLLED in other schools after finishing the 5^th year BS in Pharmacy major in Clinical Pharmacy

1. Photocopy of Board Rating (if registered pharmacist) (scanned copy)
2. Original Transcript of Records (scanned copy)
3. Resume with picture (scanned copy)
4. Original PSA Birth Certificate and Marriage Contract (if applicable)
5. Photocopy of PRC ID (If licensed RPh)
6. Photocopy of Board Certificate (if applicable)
7. If US- Visa holder- Photocopy of Passport
8. Honorable Dismissal
9. Good Moral Character
10. two (2) pcs. of 2 x 2 Picture

Note: Enrollees holding foreign passports must appear personally for Immigration purposes, as required by the School Registrar.

1. Submit Initial Documents

4. Wait for Enrollment Approval

Notification will be sent once enrollment is approved.

5. Pay the Necessary Fees

Payment can be made either online or onsite.

6. Confirmation of Enrollment

You will receive an email confirmation indicating that you are officially enrolled.
